If you have requested that we adminster medication to a child, we will require written consent from the parent or legal guardian for all medications including prescription and non-prescription. The consent form includes details about the medication, such as the name, dose, time, and method. All medications should be given to either Kelly or Ruth at the beginning of the session and will be kept in a lock box while on site. If your child has an ongoing medical condition, we would help to create an Individual Health Care Plan with the parent or legal guardian and in line with specific instruction from medical staff.
